Walk For Life Registration

Please fill out the form below to register for the Walk for Life. You may also download and print the registration form to register by mail by the following links- Registration form for those raising pledges and Registration form without pledges.

Where: 703 Main (Pathways Office)

Registration Fee

$21 before May10, 2013 (includes shirt)
$26 after May 11, 2013 (No guarantee of shirt)
Kids 12 (accompanied by adult) and under free (no shirt)

Pledges

Level 1: $500 and up: Free registration for you and your family. 2 shirts, eligible for grand prize drawing.
Level 2: $350 -$499: Free registra-tion for you and your family. 2 shirts, eli-gible for select prizes.
Level 3: $150- $349: Free regis-tration for you and your family, 1 free shirt, eligible for select prizes.
Additional shirts may be purchased for $15 (please order in advance)

Open House of Pathways of Pella will be held before and after the walk.
